Dear Friends,

 

Greetings!!

 

The last month we saw a spate of discussion in our network. Some of the issues we covered are - operationalising gender sensitivity in maternal health programmes, identifying motivating factors for ASHA, anemia control and utilization of untied fund under NRHM and Active Management of Third Stage Labour.

 

In August, like every year, we celebrate the Breastfeeding Month and discuss breastfeeding issues extensively. Marking this occasion we have published our first full fledged newsletter. However, to make it a regular feature of our community, we need to formalise the process and we request you to give your feedback on this. We find that it is feasible to bring out issue specific newsletters once in every 3 months. We will send you specific questions in a separate email about the newsletter.

 

Coming back to the breastfeeding month, please send in your queries and comments on breastfeeding issues, this year focus being mother support.

I am quite surprised that very few colleagues in the MCH community interacted on this issue! I had a feeling that all will rise to the call ('cry' may be more appropriate) for increased funding for the maternity care & to ensure quality of care. No responses addressed the problem of woefully inadequate quality of maternity services in public hospitals, or even in so many private hospitals. At no point of time did I want to push up for the public private partnership. Neither did I want a comparison to a foreign country. What I really wanted was a response as to what, in our mind or in calculations, would be a fair value of quality maternity care, in India. I showed what Govt. of Gujarat feels fair. Swedish example was a case in counterpoint, to show how much value someone else puts to the maternity services. It was indeed a mega surprise for me that not even one response came through asking for an increased funding - be it private, or be it public! In fact someone even mentioned that it can be cheaper than what it is! I wanted colleagues to come up with a mathematical reality to consider the value! Perhaps I could not explain myself clearly, or may be I am asking for something which is 'not worth' the thought! I salute the community members for their attention & space provided for the discussion.

In messages to mark World Population Day, 11 July 2008, Secretary-General Ban Ki-moon and the Executive Director of the UN Population Fund (UNFPA) noted that improving maternal health – MDG 5 – is lagging the furthest behind among all the targets. He noted the three basic interventions necessary to improve maternal health: skilled attendance at the time of birth, facilities to provide emergency obstetric care and family planning.

Motivation is required at all level. May be it FP or JSY. We can say it as incentive or bribe. In a present case in Orissa the motivation by the media which has able to stop female foeticide. Exactly a year back, the whole State was rocked when the rampant female foeticide incidents in Nayagarh district hit headlines. However, the number of female children in the district has remarkably increased within a year, thanks to the impact of the media hype of the cases of foeticide.

 

 Seven female foetuses were first recovered at Duburi hills in Nayagarh, which became news in several print and electronic media. Subsequent to a series of media campaign, social activists and Government agencies swung into action and a Crime Branch probe was ordered.

 

Doctors involved in illegal sex determination practice were suspended and a series of raids conducted upon private nursing homes flourishing in the area. Thankfully, after one year, a sea change was witnessed in the female sex ratio in the district. As per the present estimate of the district administration, per 1, 000 male children the number of female children has increased to 970 as against 910 during the occurrence of the incident.

 

Similarly, 1,988 girl children have been born in 2007-08 as against 1,546 in 2006-07, the estimate said. This clearly speaks about the change of mindset among the people of Nayagarh district, who were supposedly considering girl child as a burden for them.

Our Group is a part of the Indian Academy of Pediatrics, particularly concerned with all aspects of Child Welfare and Development.  Every year 27 million babies are born, but only a small proportion gets proper care.  A burgeoning population if of serious concern and has impeded the development and the quality for the majority.  Jansankhya should have been stabilised at least 25 years ago.  But in a democratic and free society there are limits to what can be done.  Our Group is based at Delhi.  Dr Rajeev Seth is the Secretary of the Group.  We would be happy to work with you and offer all help.  We are paediatricians, but have a deep concern for the total welfare of children (not merely care of the sick child).
